Field supervisors and cooperating teachers use the Short and Long Formative Assessments to evaluate student performance during the Intern semesters and at the midterm of the Student Teacher semester. Students also use the form during the same periods to conduct self-evaluations. Log on to the Online Evaluation System (OES) to complete the forms. Formative Assessment Form Templates for the OESFormative Assessment (Short) Use the "short" form at the midterm and/or the end of the Intern I semester, and at the midterm of the Intern II semester.
Use the "long" form at the end of the Intern II semester, at the end of the Intern III (SED) semester, and midterm for the Student Teacher semester. |
