BEFORE the SP or SU Semester Starts (Faculty advisor: Dixie Stanforth)

  1.  Meet with your Academic Advisor and initiate the ADVISING FORM.  This step ensures you qualify for the internship (class status, GPA, etc.) and they will send a copy of the incomplete form to you and Dixie Stanforth with advisor approval to begin the process. If you are planning to complete a cardiac rehabilitation internship, you must contact Phil Stanforth after meeting with an advisor prior to beginning the approval process for additional information.
  2.  Where do you find if your SITE is an approved site?  Check here: Internships: Approved Sites
    If your site IS approved, fill in the Affiliate Program Agreement & Educational Experience Affiliation Agreement approval numbers on your advising form prior to turning in.
    If your site is NOT YET approved, you MUST share the links as explained on the Internship page above and your site must be in the process of getting approval when you register.  Include the date this was submitted on your advising form prior to turning in if the site has not yet been approved.
  3. Submit the Student Internship Agreement form via DocuSign
    1. Click the DocuSign link above.
    2. Enter your name and email address and then click "Begin Signing".
    3. Fill out the form, sign and initial, click "Finish".
    4. A copy of the completed form will be sent to you and your Faculty Supervisor.
    5. Download a copy of this form to save in your files.
  4. Submit the Internship Supervision Contract form through DocuSign
    1. Click the DocuSign link above.
    2. Enter your name and email address, your Site Supervisor's name and email address, then click "Begin Signing".
    3. Enter the Semester field, then click "send".  (The Form will route to your Site Supervisor to complete)
    4. When you receive an email back from DocuSign, click "Review Document", read carefully and sign the form, click "Finish".
    5. A copy of the completed form will be sent to you, your Faculty Supervisor, and your Site Supervisor.
    6. Download a copy of this form to save in your files.
  5. Submit your ADVISING FORM: Email the COMPLETED form to your Faculty Supervisor (d.stanforth@austin.utexas.edu). Please ensure that ALL fields are complete (semester, unique #, site information, EEAA/PA approvals, etc.) Name the PDF as follows: Last Name, Semester, Class, Unique (Example: Stanforth SP20KIN62711115) 

    You will not be able to register for the class until steps 3-5 are complete.
