The following fields are necessary for a conservation assessment and treatment record. An assessment is performed by a conservator for the purpose of evaluating the need for conservation intervention or to prepare an conservation treatment proposal. If a conservator proceeds with a proposed treatment, the activity type may be update to reflect that more than an exam occurred.
Activity Type: Conservation Assessment, or update to Conservation Treatment
Review Date: date of assessment by conservator/technician
Treatment Proposal date: date a treatment proposal is received
Request Date: date a treatment is approved to proceed
Treatment Completed Date: date treatment activity is reported as complete
Assessment summary: add brief description of assessment and/or treatment
Use text entry fields to add treatment proposals and treatment reports
Link self and conservator and update each role to reflect staff title, or role in the activity.
